So that you can prepare for your first visit with us, simply fill out the medical history form at home and bring it with you to your appointment.
Transfer form
As dentists specialising in the fields of periodontology, endodontology and implantology, we offer you the opportunity to cooperate with your family dentist. Depending on your wishes, we can carry out treatments in one or more of the above-mentioned specialities. The treatment will always be limited to the scope of the referral order and you will be referred back to your family dentist after the treatment has been completed.
